WATER IMPROVEMENTS PROJECT 2023 - TANK PHASE

CONTRACTOR PRE-QUALIFICATION PROPOSALS

Introduction

Daggett County is requesting statements of qualification (SOQ) from qualified contractors for the construction of the Water Improvements Project, 2023 – Tank Phase. This phase is to pre-qualify contractors based upon statements of qualifications. Contractors that are pre-qualified will be invited to participate in a subsequent cost bidding phase where the project will be awarded to the lowest responsible, responsive bidder.

The Project is expected to bid in late summer 2023 with a flexible construction window through late fall 2024.

Project Description

Schedule A: Construction of 1,000,000-gallon concrete tank and all associated features. A valve/meter vault that will include all valves, meters, and pipes for inlet, outlet, and overflow lines. Piping outside of the vault for the inlet, outlet, and overflow lines, will range from 6”-12” pipe with a total of about 630 feet.

Funding for the project will be provided by Daggett County, the Permanent Community Impact Board and the Division of Drinking Water.

Construction is anticipated to be completed in 2024 (although it could be earlier depending on contractor availability) depending on contractor and materials availability. The project is expected to have a 120-calendar day contract with a start date to be negotiated with the selected contractor. Completion of the project is expected by November 1, 2024.

Proposal Format

Proposals should be presented in a bound format using 8 ½ x 11 sheets and should be prepared according to the outline below. Proposal page limit shall be 12 pages excluding cover and Appendix.

1. Contractor Introduction

a. Provide a brief history of the company.

b. Outline areas of expertise.

c. Summarize applicable licenses held by Contractor.

2. Bonding Status

a. Name of Bonding Company?

b. How long has Contractor been with Bonding Company?

c. Bonding Capacity?

d. Has your company ever had to file on a bond? Explain.

3. Relevant Experience & Qualifications

a. Provide brief resumes of key personnel to be assigned to project including relevant project experience.

b. Profile past projects which demonstrate Contractor’s experience and capabilities.

c. Outline critical or challenging components of the project and explain how your experience and qualifications will help address these items and provide benefit to the project.

4. Workforce, Availability, & Schedule

a. Describe the workforce, equipment, and resources maintained by the Contractor which are necessary to complete the project.

b. Detail the available workforce, equipment, and resources that can be committed to this project if the project were to be awarded to Contractor.

c. Include a statement indicating Contractor’s ability and commitment to complete the project by the November 2024 completion date. Provide a preliminary schedule for project completion.

5. References

a. Provide a list of references including entity, contact names, phone numbers, and email addresses.

Appendix - Provide a brief listing of projects completed by Contractor within the past 5 years. Include similar Tank projects.

Evaluation Criteria

All proposals will be evaluated and ranked according to the following point system.

A. Company History and Bonding Status- (20 POINTS AVAILABLE)

a. 15 points based on bonding capacity over $3 million per project, and aggregate bonding capacity over $15 million.

b. 5 points based on contractors’ years in business.

B. Relevant Experience & Qualifications- (30 POINTS AVAILABLE)

a. 15 points based on applicable project experience.

b. 15 points based on when applicable projects were completed.

C. Workforce & Availability- (30 POINTS AVAILABLE)

a. 10 points based upon availability to complete the work no later than November 1, 2024.

b. 20 points based on listed staff qualifications directly relevant to applicable projects completed and years of experience.

D. References- (20 POINTS AVAILABLE)

a. 5 points based on listing 3+ references.

b. 5 points based on references pertaining to applicable projects.

c. 10 points based on references contacted.

Please note that minimum requirements for pre-qualification include sufficient bonding capacity to complete the project and the ability to meet the required deadline. SOQ’s not meeting these minimum requirements will not be considered further for pre-qualification. A committee of Daggett County commissioners, Sunrise Engineering, and Daggett County/Dutch John Water staff will review the SOQ’s and grade them in accordance with the listed evaluation criteria. SOQ’s not meeting the minimum score threshold of 90 total points from the list will be disqualified from bidding on the project.

Pre-Bid Meeting

Those contractors who are pre-qualified will be expected to attend a pre-bid meeting. This meeting is anticipated to be scheduled for the end of July 2023.

Submittal

Six copies of the statement of qualifications shall be submitted to Daggett County before 10:00 A.M. on July 13, 2023, at the following address:

Statements shall be submitted in a sealed package clearly bearing the name of the Contractor, address, and title of “Dutch John Water Improvements Project – Tank Phase Statement of qualification”.

Notes & Disclaimers:

All Contractors shall be licensed to conduct business in the State of Utah.

Pre-qualification packets shall be completed in accordance with the submittal instruction contained herein. Failure to comply with the instructions may result in rejection of the submitting contractor, at the sole discretion of the selection committee.

Daggett County reserves the right to reject all pre-qualification packets. The County also reserves the right to waive any irregularity, informality, or technicality in the proposals at its sole discretion.

All costs related to the preparation of pre-qualification packets and any other related activities are the sole responsibility of the applicant. Daggett County assumes no liability for any costs incurred by applicants throughout the entire selection process.

All pre-qualification packets, including all attachments, supplementary materials, renderings, sketches, addenda, etc. shall become the property of Daggett County upon submittal by the applicant and shall not be returned to the applicant.
